2018年11月，威肯在表演途中向观众透露“第六章”即将来临，以揭示正在制作新专辑[9]。其后威肯于2019年发行多首合作单曲，并在8月6日向歌迷再次确认正在制作他的第四张录音室专辑[10]。在经过一轮沉寂后，威肯的单曲《Blinding Lights》于11月24日在梅赛德斯-奔驰的电视广告中率先亮相[11]。一天后威肯在封面照片流出后公布了《Heartless》的发行消息[12][13]。歌曲其后于2019年11月27日在威肯的“别忘记你终有一死”Beats 1（英语：Beats 1）电台节目第七集中首次公开[14]。在接受《CR Men》的访问中，威肯确认了《Heartless》是他自2018年的迷你专辑《致亲爱的忧愁（英语：My Dear Melancholy）》后第一首为第四张录音室专辑《黑潮时刻》创作的第一首歌[15]。

《Heartless》的歌词灵感来自于威肯的前女友贝拉·哈蒂德跟赛琳娜·戈麦斯。在歌曲的第一节中，威肯唱出自己跟她们分手后回到原本的花花公子生活模式。在歌曲的后半部分中，威肯再唱出自己与哈蒂德离离合合的恋人关系[16]。
